I'm just lost on why polyps are falling outs there's still a whole polyps, they just fall out of the skeleton still look healthy and all. Confused and frustrated

I lost my Golden wall hammer over the span of this past week. Just receded till it was nothing but a skeleton.

a friend of mine used rodi water, dry rock , new salt, new tank etc, let it cycle, added fish 2 months later, all was well,
then when he added coral, they would only live for a couple days and it would die, all levels fine, i went over and personally tested them,
i was baffled and so was he, so he took a sample of water into a water store somewhere in hamilton and turns out the amount of copper in the water was insanely high..
maybe get that tested pat? and tony?
its not something we normally check for so may have gone undetected..?
again, just trying to brain storm with/for you guys.
